Does anyone have an ACO care plan form created? I really did not want to use the care plan available in GE as it is not as user friendly.

Our providers use the html care plan, but we needed a way to give the patient a hard copy that made since. So I created a very simple care plan form that list active care plans and others can be saved. It could be used just to enter them as well. I also have a simple letter to print out. If its something you might be interested in leave me your email.
