Helping another group with a report they accidently deleted. It is basically the appointments report but they want provider and date as header, then time, patient name, patient id, appt type, status, referring provider and referring provider phone number and appointment notes. They would like patient separated with lines. It has been entirely too long since I worked in crystal and dont have a lot of time to work on it. Anyone have something like this they are willing to share?

Just a thought, since we had this happen as well, do they have a back up copy? We were able to find our deleted report in a copy we had made prior to an upgrade.

unfortunately no. I was really hoping we would find an old copy but no such luck. I think it must have been created when they went live. They say 'we have had it for years'. thanks for the idea
