We currently use the CPOE A&P-CCC form. I'm looking for anyone willing to share their VFE version of this form, or one they use for Assessment & Plan. I would like to view how others address this part of the clinical workflow with a non-GE form to evaluate the merits of switching us.

The Assessment and Plan is arguably most critical section of the provider note. The A/P should be where providers synthesize interval history, interval data and current exam and update and add problems accordingly to end up with a problem list that most accurately reflects the current understanding of the patient. I had been frustrated with the CCC CPOE AP for years and finally engaged the help of Jonathan Rodriguez from EHealthSolution [email protected] to completely redesign that element.
Our AP now includes a full view of all pt problems, sortable by last assessment date, with easy view of last assessment AND problem comment (critical and sorely underused). The form includes direct access on the same screen to quick add pt instructions at the same time as updating assessment. It is a dream come true. The assessment and plan section is built in as part of an entire visit form but could likely be extracted for use as separate form element if that's what your are looking for.
There has been a ton of work that has gone into this so I don't think it would be a freebie but well worth contacting him to see and hear about options.
