I am search of a Care Management Plan form. Does anyone have one they are willing to share?

What version of CPS/CEMR are you on? I ask because CCC Basic 1.5 comes with a Care Plan Management form that our Care Plan Team just fell in love with.
If I remember correctly, I think CCC Basic 1.5 is only compatible with CPS12.3.2 and the latest version of CEMR.

We are on CCC Basic 1.4 and it does include a Care Plan Management form but it does not fit our needs. Our Health Coach w0uld like a form that includes a time keeping feature.

Sorry, I forgot that there was an earlier version of a Care Plan Management form in the CCC Basic Package prior to 1.5. The new version in 1.5 is actually an HTML form, but I don't believe that there is a time feature. I do see that our users use a Quick Text in the intervention field to mark who wrote the intervention and the time that they wrote it. I would think they probably can do this with the comments field too.

Here is my version of a Care Plan Management.
Also a Chronic Care Management used in conjuntion with a special phone note with a timer to keep track of time on a call with a patient.
