I'm in search of documentation and instructions on the Multifactorial-CCC form and use. I've searched through what manuals I have without any luck.

It is included in the CCC install package. You can download the 8.3.7.3 package from GE's site (or you may already have a CCC install package). Look in Documentation\User Manuals for 8.3.7 (or whatever version you have) and it is in there - Word document. If you still can't get a copy I can turn it into a PDF and upload it.
