I am wondering if anyone out there has a version of the DAST or a SBIRT Depression screen that they would be willing to share. We are looking to get these forms into our EMR so the screens can be done electronically, including the documentation.

Hi there.
We had a program in 2014 where we did alcohol and substance screening; each form had a one-question to pre-screen. We did the full tests on paper, and then recorded the result score in the component. We also performed the morisky for med management.
here are the two handouts - you will need to copy/paste into GE handouts and then modify the paths for the form buttons.
