We are going to be bringing up our first specialty clinic, Endocrinology, very soon. I am struggling with resources for our Nutrition and Diabetic Educators to use. They are Registered Dietitians and have certain guidelines they are suggested to follow for their assessment that do not match a typical office visit, is more detailed than the Diabetic Q & E form (especially when they are seeing non-diabetic patients) and follows a process like this:
Problem identified > related to > as evidenced by...
An example would be assessing a patient with a problem of Excessive oral food/beverage intake related to inability to limit or refuse offered foods as evidenced by anthropometric measurements/BMI.
The RDs gave me a list for each segment of the assessment process. I could create (or have someone create since I have not had training yet) some sort of EFE form with check boxes that text translates, but if someone has something out there already and is willing to share, that would be awesome!
