We had a call with MQIC yesterday. They mentioned that to meet a measure, a patient needs to be seem twice in a 3 month period. Has anyone else been told this, or may I have misunderstood ? This would not be something that would get many hits, as most patients only get seen every 3 months. Any feedback would be appreciated. Thanks.

The reports are correct as are you. Two-or-more certainly limits your numbers in a 90-day period (and can hurt specialists chances of having a denominator).
Many MU reports (based upon NQF) or NCQA measures have two-or-more visits in the criteria (some also require the first visit to be >1 year ago). In that logic, you are more likely to be 'managing' a patient.
