I am looking for people to send any diagnosis forms they are willing to share. We have built many types and varieties, yet our providers find them cumbersome and tend to not use them. In light of ICD-10 and the disarray of our problem lists, we really need to get them to find one they like and use it. What they really want is one that will transcribe itself to their CPOE and even their orders. My magic wand is out of juice
