We are interested to learn how providers are notifying the pharmacy when a patient's medication that still has available refills has been discontinued. For example: Is this being done electronically? Is the provider/nurse placing a phone call to the pharmacy and documenting that conversation in the EMR?
The concern is the pharmacy has the medication with available refills still on file but doesn't know the provider has stopped the medication. Therefore when the patient calls in to have their meds refilled they are potentially obtaining a med that is d/c'd.

We would like to know this information as well. We have a request in to make some sort of change to either Prescriptions or refill form to somehow notify pharmacy or document that they have been notified.

One of the options that we have put into place when a patient medication has been changed, is in the comments section of the Rx "discontinue ____" and in the notes to pharmacy. "Please void refills on _________". This seems to have helped considerably.
Of course this only works when changing a medication. We have not yet found a way to just discontinue the refill requests without a medication change.
