Anyone have EMG/nerve conduction study forms they may be willing to share?

I'll get that information from the developer of that form (he is out today) and I'll upload the kit.

David Shower - I was wondering if you might have the custom OBS terms that the Neuro forms that you had posted here? They look to be very useful forms for us and I'd really like to be able to do some testing with the OBS terms if you wouldn't mind.
