I didn't obtain the form and files for the End of Visit form created to meet MU for the clinical visit summary. I meant to get an email address or something while I was at the CHUG but forgot about it. I think the form was presented by Joel Berman or Dr. Joel Berman of Concord Hospital. Please send this to me if you have it from the Spring 2012 CHUG. Thanks

Ernie, thank you very much for these forms, that helps a lot!
However the handouts that are linked to the checkboxes are not ones we have. Were those also provided at a CHUG? Could you provide those handouts, also?

No they were not. They are sort of there as a place holder for your own handouts. You would need to modify the form to point to the handouts you would like to use.
