We are so fed up with the FH/SH CCC form. Does anyone have a good VFE form to capture Family History and Social History?

Here's our for mu 2 you'll have to modify the code and visibility region. We basically have it set to the user home location what they'll see or they can select it.
Didn't CMS relax this requirement earlier this year (2015), basically saying that they believe it is begin done by practices and thus does not need to be recorded? Thus, no longer a need for the very complex process (form) that GE supplied to create the FH data.
Joe - the proposed rule for MU stage 2 (which I 100% believe will mirror the final rule) removed the requirement to report on family history. For this reason we did not burden our providers with the requirement of using that form but it is in our form sets now for those they may want to use it.

This form is also enabled to connect to Visual Signature Capture to gather data directly from patients, eliminating key entry by front office staff and minimizing key strokes for providers and clinicians.
