Is there anyone that has found or created a form to use for a trigger point injection given in the office?

I have a Trigger Point injection form that I have built, it does quite a bit. Pretty much 3 clicks to document a procedure - choose from approved diagnosis, Enter the Trigger point location, click to enter - then multiple options available for medication used during the procedure.
