Back in Feb 2012, I opened a case with GE regarding the Patient Authorization form. The "Authorized By" drop down wasn't pulling correctly. I think they said it had to do with the LOC of the patient in reg? I also asked if you could add contact relationships.
The answer given was: This is an EFE form and you can modify it if you want.
Has anyone modified this report to "fix" the Authorized By? Can you send me the clinical kit if so? I don't have EFE.
Or, do you have 2 cents on this topic?
PS: I was looking at the old CHUG archives and it looks like someone redid this form, but the archives will not let me download the zip file.

I have never been able to get this form to work. Did you figure out how to address the "Authorized By" field? I opened a case with GE back in January and was told it the form was defective, but I did not get any help in a fix. Is there anyone on CHUG using this form successfully, or had to do modifications to get it to work?????

When you say the "Authorized By" drop down isn't pulling correctly, what do you mean? On our form the drop down works but we will have our docs listed in there anywhere from two to four times. Is that the same problem you have encountered?

I can answer, that we do not get any value in the drop-down list...completely empty!
