Has anyone built a form for a Global Pain Scale? Our pain mgmt doctor wants this built so he can track it with each encounter. It looks like it came from paindoctor.com and it also has a copyright from Lynch, Gentile, McJunkin & Woodhouse in 2007. What is the protocol when a form has a copyright? Is it not available to be built or do you need to contact them form permission or do you just give proper credit? We aren't really sure what to do here if we end up having to build this……. Thoughts?
Update*** I built the form with the appropriate credit and put the copyrights on as they have the form available for print and download to use freely.
