Does anyone have a good HEDIS form?
Ar you looking for something to enter in data, or to review data?
Both enter and see what is previously entered
Here is the VFE form designed earlier this year, for some of the key (to us) HEDIS measures.
A screenshot is below. You may need to tweak some parts, as some pull-downs may default to Tufts.
It displays most recent data, and allows for entry of any-date items. Enter value, data and then press the button to commit. This is because by default, the date of OBS as part of a note take the overall Note date. Thus, an MA can enter a few missing items prior to the patient being seen by the doctor.

just right click his link and save as, change the file type from html to "all" and it will save as a dlg

I am late to this game and I just had a PM come ask me to build a form. I have tried several different ways to get this link to save so I can open in VFE but it is not cooperating.
