We recently upgraded our test environment to SP 5 on CPS12 and how our HTML forms (med admin and immun) are not working. Has anyone else experienced this? It may be that we are missing a file in the Client folder. Our CVS is working just fine.

My guess is that the re-installation of the website during the service pack clobbered your CCC basic html files located on your jboss server usually in c:Program FilesCentricity Practice Solutionjbossserverdefaultdeploympc.earCentricityPracticeWS.warEncounterForms. You should see the Immunization and MedAdmin directories in here. If not, you need to copy them there again.
You should be able to obtain them from the zip file in your CPS 12 staging directory – usually something like c:CPS_12_StagingClinkitsCCC_Basic on your jboss server.
In the same zip file, there is also a "Copy to Client" directory that supplies the files for the fat client or terminal server.
I am not sure why GE can't build it into their install routine to copy all of this to the appropriate spot.
