While it looks like we are using the standard injection form (I can see the load comments form 2001), probably built with EFE, I have been asked to modify it. Since the form name does not always have to match the file name, I cannot seem to locate the original form.
Any ideas on name?
Any ideas on how I could obtain another base file?

I may not be understanding your post correctly, but if I am, it could be that the form was loaded from a clinical kit that your organization was provided, and clinical kits don't necessarily include the actual files needed for editing.

Ouch. Since I was asked to add/change a couple, thought that I could simply edit the specification file (even if it needs to be up-converted from EFE to VFE).
Assuming my cut/paste displays, the following is my current injection form.
Would you try attaching it in a separate file?
Sorry - I don't recognize that particular form, but its design if familiar; someone else on Chug will probably recognize it. It is my understanding that you'd definitely need to have either a .dlg or both .fs and .fd files in order to modify it.
Version 5 of VFE can bring in a Clinical Kit and most of the time convert it to a dlg file. You can export the form and try that.

I just cannot seem to locate any original source files for this form. It appears that sometime after we originally went on-line with GE, there was someone who loaded, modified and created forms (2001-2003). And I am working from all of her old directories, but cannot seem to track down the original source for this.
Without it, I would have to build-again from scratch; and it looks rather involved with pop-up or visibility regions as the user clicks on the left-side boxes for different injection types.
Thank you, David. That is really helpful to know; I hadn't caught that. Now I'm wondering what other aspects of the program I've missed along the way.
The Clinical Kit import only started with version 5, so it hasn't been around that long. I think this is the clinical kit from the original Injection form. The comment in the kit says "Documentation form for single or multiple injections 2/1/00; Authored at: Medicalogic, Site # 1 on 4/7/00 3:20:12 PM with 2.1.3_3"
I think I've found our enterprise's original files related to the injection form, though the date stamp is 2003. I can send you screenshots in an attachment for you to look at, if you want, and then if it looks right, I'd be glad to zip you the whole folder (in fs and fd). I have no idea if the visibility fields and so on work, but it may be worth checking. Just let me know if you want to take a look.
