Does anyone have a form that will produce a Medication Administration Record? We have a Mental Health clinic serving some severely and persistently mentally ill patients who need to come in every day/week to get their meds. Our pharmacy partner produces MARs for us each month but they have to be updated by hand for mid-month changes. We'd love to find a way to have the EMR produce a MAR based on the patient's Med List.

Turns out this form is designed for RNs to chart primarily IV meds administered at bedside. (Or at least that what it seems to me.) Each med administered has to be manually entered.
We're working in an outpatient setting, with the RNs administering meds that have already been ordered by the psychiatrist and are on the med list. Sometimes pt med list has 10+ items so wanting to avoid manual entry. Hoping for form that pulls from med list, including instructions, to create a MAR. Eg if Wellbutrin 150mg is ordered TID then the MAR would automatically create a line for Wellbutrin 150mg with 3 spots to document administration of each dose.

I am working in a practice similar to yours in CT and was wondering if you had any success with the MAR for oral or outpatient medications.
We are limited in our forms creation capacity so if you had anything to share it would be great!
