I've been getting a bunch of chart notes that have "MLI: 56489465469" (which is the ID in the database that links the correct form to that spot in the encounter) in the Documents tab of the chart module. So, what usually happens is the doctor will end up signing this off. So, when they want to come back and view or the billers want to take a closer look at something, there is nothing to be seen except the ID of each form in the document.
Has anyone else encountered this problem and found what was causing it? I can fix it if the document is on hold, but if it is signed, I can't really do anything.
We've found that an extra right bracket ']' in the document template causes this. These brackets appear sporadically. We run a daily report looking for these and remove them.
I've found that these literally appear again hours after I delete them sometimes. I'm checking on a template that I deleted one from this morning, and I see it has an extra bracket at the end again. You happen to know why the extra bracket is being appended to the end of some of these templates?
no clue. we've notified GE, so should be in next patch or SP.
I wouldn't count on a fix for this in the next SP since the extra bracket issue has been around for years. I filed my first SPR for this back in 2005. I've found that by adding an extra white space or two after the last closing bracket in the document template seems to keep the extra bracket from appearing again.
I suspect that the extra bracket appears when the document template is updated after an updated form component has been imported but I've never been able to confirm this.

Interesting. I'll give it a try, and thank you for your response.
