The files below are the result of development at the Vegas CHUG workshop where Lee Cooper worked with a group to develop a Clinical Summary form.
The files are uploaded zip files but have been renamed to include a .txt suffix to allow safe upload and download.
Looks great but had a few questions about it.
The handouts that are listed? Where did those come from?
Also one small thing I noticed on the counseling predominat visit, needs a space between the heading and the information. My OCD kicking in on it.
All in all, great form and handout. Thank you everyone that worked on it.
Correct Ernie, it did. I also tweaked a few other items.
The handout list is designed to be customized locally, as are some of the buttons that insert text.

The form in Centricity seems to function okay. Lee I see you posted a form that contains the source codes to the handouts. I am having problems getting to that, it says it's an invalid web page. Is there another way that you could post that information please?
Does the handout only work when you print it from the button within the form? When I try to print the handout from the Handouts module within the patient's chart all I get is an invalid MEL line.

The handout functionality is a simple set of list boxes, a set of watcher expressions, and a function that triggers the PRINTHANOUT data symbol. The list of handouts needs to be customized by the site so that it points to handouts they actually have. The .dlg file is included and the code commented well enough (hopefully) to follow.

Blanking on how to run a trace- I thought it was control M-however that is not working.

When I click add the medication list or problem list I get this message. What was the reasoning behind this?

I would like know how people are incorporating this form into their workflow? Are you having nursing fill the form in at the beginning of the office visit and then the provider finishes and prints at the end. Who ends up at the end printing them out and handing them to the patient. Because for example if immunizations are given those are not given until the end of an office visit. So the nurse comes back in after the provider.
