Hello,
Does anyone have a Family or General Practice Medical Assistant intake form they would be willing to share? Our MAs are in so many different forms trying to populate risk factors, social history, screening tests, etc when they take our patient's back. We are a small, four physician, office, and we do not have the resources to make a custom form right now. I would like to see what other offices are using in hopes one of them could work for us.

Even if you do not have the resources to build your own custom forms, you may want to take the forms you are currently using and put them together to create an encounter. When a patient checks in, you can have this encounter start up and then the medical assistants only need to follow the flow of the forms they are in. Just a suggestion.

Our medical assistants basically only use 2 forms. We are a specialty office, but I have attached our forms for your review. Please let me know if you have any questions.
