Our providers are wanting me to build a form for Medicare Wellness Exams so that we can stop utilizing the paper forms that are required for insurance to pay for these exams. Since this would/will be a fairly large form to build I wanted to check here first to see if anyone might already have some that would work for us. I would be looking for a form for the Initial Preventative Physical Exam (IPPE), the Medicare Annual Wellness Visit and something for medicare pap/breast exams.
Does anybody have forms they would be willing to share? Let me know if you need any additional information from me.

This is something I was just given also so if anyone has already one created that would be wiling to share it would be really really apreciated.

I have built a Medicare Annual Wellness visit form based on our Community Health Center's old Paper Form. I will link the file here and you can feel free to modify and use it if you would like. Please email me if you have any questions or issues with it.

Hi I am new to this site, I hope I am doing this correct. My Nam e is Crickett and I work at Sunshine community health center in Alaska. This subject just came up with the medicare wellness. I would love it if I could get you to share that with me as well. Thank you in advance.

Welcome to the site Crickett! Glad to see another Community Health Center on the CHUG forum.
I linked the form as a .zip file in my original post. Just click on the blue words that say "Medicare Annual Wellness Visit" and it should download the files to your computer. from there, you can edit it with VFE, or stick the form straight into Centricity.
