This seems like a needle in a field full of haystacks, but my Medical Director would like a form to calculate a patient's daily Morphine Equivalent. Anyone have any ideas?
Also, wondering if anyone has an Opioid Risk Assessment, DAST-10, SOAPP-R or similar that would score itself...
Would appreciate any help!

I did find a web based opioid calculator but I would like to incorporate this into a custom VFE form. Has anyone found or created a custom form for this?

Along these same lines does anyone have forms created for the following to do Opioid Risk Assessments:
- Diagnosis, Intractability, Risk, Efficacy (DIRE)
- Addiction Behavior Screening
- Current Opioid Misuse Measure (COMM)
My office has been free-typing these (using Quick Text). I am hoping to have the info entered as structured data.
