I am looking for a Calculation for Caloric Requirements and Initial Insulin Dosage.
It would look/be something like the picture below. This came from Perinatology.com.

Guidelines for the Calculation of Initial Insulin Dose
Weeks of Gestation | Total Daily Insulin |
Week 1-18 | 0.7 U/kg actual body weight |
Weeks 18-26 | 0.8 U/kg actual body weight |
Weeks 26-36 | 0.9 U/kg actual body weight |
Weeks 36-40 | 1.0 U/kg actual body weight |

So for this patient the Total Daily Dose = 54units
2/3rds given in the am and 1/3rd in the evening split 50:50
36units in the am, split 1/3rd regular and 2/3rd NPH (12/24)
18units in the evening split 50:50 ( 9u regular ac dinner and 9u NPH at HS.
We would switch our protocol to reflect weeks gestation and insulin per unit/kg
