Hello,
I am in need of the following forms for Opioid Risk Assessments - is anyone willing to share or can you let me know if they are offered somewhere?
- Current Opioid Misuse Measure (COMM)
- Addiction Behavior Screening
- Diagnosis, Intractability, Risk, Efficacy (DIRE)
My office has been using quick text do document these, and I would prefer they go in as structured data.

I have an opioid risk assessment tool. I would be delighted to share it with you. If you need additional tools, I can develop them for you. Please contact me directly and I we can determine if the existing form is applicable for your needs.

Here's the dlg for what we use.
The original was pulled off of here and I added a MME calculator within the form. There is quite a lot going on and the form shows it's sluggishness.
