We are also looking for a template to use for care management for PCMH tracking. What three conditions did you choose? We'd love to take a peek at the template you created!

We have will track: Diabetes, Heart Disease, Depression as our 3 chronic conditions. We will also track HTN & Hyperlipidemia for back-up. I will have to ask out IT before I download the program needed to post the template. I can send you the screen shot of the file if you want to take a look.
