Does anyone have any care Plans forms. Has anyone been successful in creating a care plan workflow we using GE.
There is a prenatal care plan that actually looks pretty good has anyone used it.
I created 1 that has Care plans self management goals and medication adherence all in 1. I present it at the aafp on Fri. If they have any discussion or changes I can share it with you. It also has a handout that goes along with the form

We are interested in Care Plan workflows or any Clinical Kit / VFE form that anyone would be willing to share. The new PCMH requirements are fast approaching, and we do not feel comfortable that the Care Plan button in the CPOE form from GE is capturing what we need. It does not really provide a plan, per say. Any information would be appreciated. Thanks .
