Does anyone have a VFE form for PEG Pain Screening Tool form that they would be willing to share with me?

Here's a form we developed that includes the PEG score. Note the obs terms for the initial PEG score is different than the "on-going" PEG score b/c our providers wanted to compare the PEG scores to the baseline score.

nthomas - The form I downloaded from your link is for Diabetes Management. Am I missing something?

I just opened this form up and it looks like it is a diabetes management form. Is the PEG screening somewhere on this form that I am not seeing?
