We have been using the PatientLink PHQ-9 card for several years with very good results. The patient completes the card and when it is scanned and opened into the interpretation document, the score is completed and the information is in a concise readable format.

I work for Logical Innovations. We are the creators of Visual Form Editor. We have several clients using custom built VFE encounter forms scoring results in the EMR. We have built some of these forms for our clients, some have been built by our clients themselves.
Many of our clients are now using Visual Signature Capture to move patient data from a tablet computer directly in to the encounter form in the patent's chart where it is auto scored.eliminating paper and labor to get the info into the EMR. (we can also score the template before the we move the data into the chart) This also works well for screening. I.E. when the patient fills out the PHQ 2 and we can detect a positive screen and auto-load the PHQ-9 for the patient to complete.
