Our clinic has asked to have the paper version of the 'Pain Assessment and Documentation Tool (PADT)' converted into a form to use in CPS.

Has anyone done this already and if so would you be willing to share your form?

Joan have you found a form that works for the PADT documentation yet? We would like to start using it in GE as well.

We decided to go in a different direction and never did the PADT documentation as a form.
