Does anyone have a form that they would be willing to share that captured all of the PCMH (Patient Centered medical home) measures for 2015 standards?

Hi Nicole,
are you on CPS or EMR? I am building for another client of my (EMR already done), the HSRP and DRP reports for 2015 standards, and soon will be building the CPS version for them as well. Let me know if you are interested, shoot me an email at [email protected]

I have a reporting system I sell with custom reports to help you track PCMH, I have it for both CPS and EMR

Do these reports assume CPS is the only system or does it take in account that Registration/Scheduling can occur outside of CPS and data is interfaced in? I had found some really old PCMH reports awhile back and we are finding that they were written assuming CPS was the only system and registration was done directly in CPS. I am attempting to rewrite these very old reports to make them work in a CPS/CB environment. If there was something already available that would work in a CPS/CB world, it would save me a ton of time having to re-do these reports.

Good Morning Jim, if you are speaking about my reports I created, yes, however doesn't mean I can't re-code it to look at another database as well, since I developed it. So yes, it can work with multiple environments, i have already have it for Allscripts EMR as well. So really it can be configured based off the needs of the facility.
