Looking for any forms or templates that are used to incorporate either M-CHAT, the ages and stages questionnaires, or any other approved developmental screening tools into Centricity. Any info would be appreciated.

We have been looking for the same thing for a while and have come up blank. We are now looking at OCR of the scan and then score from there.

We have a 2-tabbed VFE M-CHAT form. The first tab has the questions and a score button; the second is all text, with an explanation of the scoring and recommendations. If you want, let me know and I'll send screenshots and then, if interested, the files.

How did you get permission to share that M-Chat form? We got permission from the M-Chat people to make an electronic copy for ourselves but couldn't get permission to share it.

That's why I didn't just post it in the first place. We have a copyright statement included on the form itself, and we are a non-profit organization, so it was ok. Still, that is a valid question that I've been wondering about for MMSE, as well.

Were these forms ever published on the CHUG forum? I would also like to see a screen shot and the VFE files, if possible.
