We recently upgraded from CCC 8.3.8 to CCC 9.4. The Well Child Check - Age Specific was removed from the CCC package starting in CCC 9.....
Our Pediatrician loved this form and it no longer works. She took one look at the Bright Futures form and said "NO WAY."
Has any one developed an Age-Specific Pediatric form and willing to share?

The Pediatric Age Specific was replaced with a Well Child Age Specific form set. Take a look at these forms to see if it will meet you need. We had the same issue and changed our Document Templates to the new Well Child Age Specific and our providers are okay with it.
