Below is a screen shot of a Diabetes Dashboard Summary report (detailed version). This report will be presented during our 'oh yeah prove it' presentation at the Orlando CHUG 2012. The report summary shows physicians how their diabetic patients are doing. Percent of patients with an A1c under 8, good blood pressure, etc. The detail report gives each physician a breakdown of each patients' details.
Diabetes Patient Base.rpt – this report runs against Centricity and dumps raw data to an excel spreadsheet.
Diabetes DB Review Loc Summ.rpt – this report runs against the excel spreadsheet that patient base report creates to summarize diabetic data per PCP.
Diabetes DB Review PCP.rpt (detail version)- this report also runs against the excel spreadsheet that patient base report creates to expand detail for each patient.
We automate these reports using Visual Cut. First we run the Patient base report which extracts data to an excel spreadsheet. Then we run the Diabetes DB Review PCP report and create a pdf for each physician using 'bursting'. This bursted report is a list of patients where that physician is listed as their PCP and all of their values; last A1C, last blood pressure, LDL, Smoking status, etc. etc. Finally we run the DB Review Loc Summ report. In this report we have added 'PDF Link' tags (Visual Cut specific) to include a push pin icon next to each physicians name with a 'link' to that physician's patient's detailed report.
