We have a form given to us (I forget from whom so I apologize for not giving proper credit) that several physicians use for CPOE. It is a great form, but there is something odd happening. When a patient has benign essential hypertension and TIA on their problem list, if you select the hypertension dx and choose your other options, when you click the add assessment button, it inserts it as TIA not hypertension. It also does this with osteoporosis and osteopenia occasionally. I looked at the coding, but it is a little over my head and nothing stood out, plus it doesn't make sense that the issue would be there since it pulls in the patient's problem list and you simply choose one to assess/plan for. Is it possible that the coding is crossed in the database? Both codes have to be on the patients list for this to happen.
Just a troubleshooting thought.... Have you looked at the ICD-9 codes for these problems? They may be the same diagnosis codes.
