We are adding a medication assisted treatment program to our practice to aid patients with substance abuse. Does anyone have any substance abuse clinical content (e.g., assessment forms, physical exam forms, etc.) that they are willing to share? Any help is appreciated! Thank you!

Couple of Crystal Report (1) to document current enrolled patients, (2) to highlight potential patients for registry
Workflow and guide documents.
Would be interested in hearing comments.
NOTE - the reports may need some adjusting on filter, as customized for our site.
