We hope our presentation at CHUG, “Taming the Handout Monster”, was helpful to you. As I have received several requests for forms and handouts, we are posting several sample materials to hopefully help you with your practice’s handout needs. Naturally, not all of the MEL code in these samples will work for you as they map to handouts and forms in our system. However, they should give you a good framework to customize.
I have uploaded three VFE forms. One is our Handout Dashboard. As you will see, the bottom half of the dashboard contains links to condition/system specific dashboards. I uploaded two of those: Well Ck/Immunization Handouts and Newborn Handouts. The newborn handout form is a pretty basic example. However, the Well Ck/Immunization form is neat in that it has buttons that link to internet sources (the foreign language VIS pdf’s).
The upper half of the Handout Dashboard has print buttons for our letters and forms, including our Clinical Visit Summary, Out of School form (includes pushing document variables from the encounter form to customize the handout), Immunization Record, medication administration letters, and our state Daycare Form. Samples of all five of these handouts have been uploaded for you including the embedded MEL. I am also including both our state Sports Form (KSHAA) and the sports form that we altered to be specific to teens with asthma.
I will also post a sample of one of our Well Check Handouts, our 4 mo ck handout. It includes code for measurements and percentiles and the patient’s immunization record.

The handouts form is great but I am having trouble with the text translation stating what handout was given. Noting is showing up in the text body of the note. Any ideas what I am missing.
