I have heard it is possible to build a table in a chart document. I need a way to organize information horizontally. Does anyone have a template with one I could look at?

Pretty easy to do. I create my table in MS Word, then cut and paste it into Word Pad (in Word Pad, put the obs terms in the boxes you want to populate) then create a new Text Component in CPS and cut / paste the table out of Word Pad and paste it in there. Add your button in your form to insert the table.
