Hello,
Has anyone developed a VFE Travel form or TeleMed form they would be willing to share. nCov form on GE website only relates to China. We would like to know if they have traveled outside of US or gotten on a plane recently.

We're quickly designing one right now and hope to have it built for testing by the end of the day. It shouldn't be too complicated.
I can share when we get it going and/or can share my design idea if you have someone who can build forms on your team.

This is our basic COVID-19 form. It's specific to our infrastructure and Oregon, but it's easily modifiable.
Here are the specifics to be aware of:
- We are waiting for OBS terms, so for now we're using a few pediatrics OBS terms as placeholders, as we do not and will never see children. When our OBS order comes in, 9 of these fields will become discreet data points
- We ask a specific question about Forest Hills Elementary, the site of our first confirmed case in OR. You likely will not need this
- We use a custom function called GetList() (which I've provided the code for) to access an external dropdown list. This allows us to be able to quickly update the list of travel locations as the virus spreads further or is contained.
We are sharing this with the full expectation that anyone who uses it will do so as a jumping off point. Take this form, modify it as needed, update parts you don't need/like, feel free to use it as a template for any other document you may like.

No problem! I'm glad we could help.
We also learned about a set of factory OBS that already exist for COVID-19. We still need to order two, but the bulk of them have been taken care of, which is nice.

Here is a form that is a merge of Ramon's and athenahealth's form. You will need both obs term clinical kits from the athenahealth's patient portal. Please let me know if any obs terms need to be changed or if there are any suggestions for improvement. I'm technical, not clinical 🙂
There is a radio button Yes/No choice of did you travel to China. If you answer yes, it jumps you to the China Travel tab and populates China as the travel country.
