An enhanced version of the Vitals Signs-2-CCC form:
Changes from stock:
Requires new height and new weight for Pediatric BMI calculation. Only new weight required for adults.
Supine BP removed- replaced with repeat BP allows providers to see previous BP and to enter second BP rather than open the serial VS form.
Meaningful Use Enhancement: Smoking as a Vital Sign for all patients over 13.
PCMH Enhancement: Care Transition: The question:Have you seen a doctor since your last visit is the obsterm CARETRANSIN which allows for the printing of the med list for reconcilliation.
PCMH High Risk group ID button. This orange button is visible when a high risk patient without self management goals. in our case diabetics, obese children,smokers, and chronic pain patients.
Team based Care: Nursing Protocols fires for flu shots. In this form we are also working to refine diabetes check-in protocols. They are only enabled at one site.
Nurse Protocols Tab
This will eventually house all of our clinical protocols that can be farmed out to our MA's.
In House Labs:
These tabs enable our clinical support staff to enter POC labs and other tests and capture charges.
