As a Licensed Behavioral Health facility we have implemented the DSM 5 and it is recommended we use the World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0).

The obs terms are in the database and I was hoping someone had the form they would be willing to share.

Any help is appreciated.
