Hi gang,
A provider in our clinic would like an Clinical Opiod Withdrawal Screener (COWS) form. I was wondering of any of you wonderful CHUG members had something on hand before I went to reinvent the wheel.
Many thanks in advance! you can google COWS form for a good idea of what it is.

I just requested OBS terms from GE to build one. Well actually I am using the shell of the PHQ9 form and just inserting the COW values. What's your email? I'll send it to you when I'm done.
